Primary Care Provider (APRN) The Primary Care Provider provides the full scope of primary care services, including but not limited to diagnosis, treatment, coordination of care, preventive care, and health maintenance to Community Alliance clients/patients. The Primary Care Provider also functions with the Practice Manager to provide overall clinical leadership and responsibility for monitoring consumer treatment and services provided by the clinic. Job Duties: • Demonstrate leadership, advanced practice skills and teamwork in the delivery of primary care services to individuals in accordance with organizational and program philosophy, goals, policies and practices and within the employee’s scope of licensure and training inclusive of: • Analyze and interpret consumers’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses. • Diagnose and treat acute and chronic health care problems. • Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines. • Recommend diagnostic or therapeutic interventions with attention to safety, cost, invasiveness, adherence, and efficacy. • Prescribe medications based on efficacy, safety, and cost as legally authorized. • Educate consumers about self-management of acute or chronic illnesses, tailoring instructions to consumers’ individual circumstance. Provide consumers with information needed to promote health, reduce risk factors, and prevent disease.

ACT Team Leader Under the direction and leadership of the Senior Program Director, an individual in this role exercises substantial independent judgment and decision-making. The ACT Team Leader has responsibility for the planning, management, and supervision of daily operations and delivery of services within the Assertive Community Treatment (ACT) team. Duties Include: Directs the provision of quality rehabilitative, recovery-focused and person-centered service on a daily basis to program participants in accordance with the Community Alliance organizational mission. Identifies and implements action steps necessary for reaching specific quantitative and qualitative program goals and objectives. Assigns workloads, staff responsibilities, and program resources equitably and in accordance with identified program and organizational goals and objectives.
