Job Description


TITLE:  Community Project Manager


MAIN FUNCTION: The Community Project Manager will function as a core member of a collaborative care team that involves the students, school personnel, families, mental health practitioners, and community resources. Responsible for coordinating referrals to clinically indicates services for clients and coordination of all training for personnel.  



  • Support and closely coordinate mental health services that include, but not limited to, case management, crisis response, training, and prevention
  • Assists in obtaining parental/guardian consent forms, releases, and required paperwork for mental health counseling for students in their homes, community, or school
  • Provides regular retention checks on all clients referred to counseling
  • Attends regularly scheduled staff meetings, clinical meetings, IEP meetings, and other as needed
  • Participates in collection of mental health/substance abuse information and student outcomes data to be used for monitoring the overall performance and effectiveness of services provided
  • Assists the mental health practitioner in finding community resources available for treatment and/or placement and makes appropriate arrangements
  • Provides crisis response on call including after hours and on weekends
  • Be familiar with disciplinary guidelines and at-risk student characteristics/needs, collaborating agency referral guidelines, as well as be familiar with special education regulatory requirements and assistance with process as appropriate
  • Foster effective communication with students, parents, and other staff members
  • Assists family to locate and appropriately use community resources, agencies, organizations and team members to coordinate services and monitor progress toward treatment goals
  • May be required to work an altered schedule and provide services in evenings and weekends
  • Coordinates all school personnel training and works closely with the student services director
  • Other duties as assigned


This position has no supervisory duties.



SALARY RANGE: $30,000-$46,000 DOE


HOURS PER DAY OR WEEK: 40 hours a week

Willing to be a team member with all staff



Bachelor’s degree in social work, human service counseling, psychology, or human services related field. Must have one year’s experience Or equivalent experience.
