SUMMARY DESCRIPTION OF  WORK

This Case Manager will serve as a direct care staff/case manager in a residential facility serving men and women with a mental health/dual diagnosis condition.   The program provides support services to individuals.  The role of the program specialist is to coordinate services to meet the needs of individuals in the facility.  In addition, they will provide direct education and support groups on site to program participants.  They will also assist individuals with daily living skills such as meal preparation. 

Intensive case management responsibilities begin at this level. Educational requirements are very specific.  Decision making, problem solving, and highly developed interpersonal skills are critical.

Work Schedule: Days of the week will vary, hours will be from 3:00p.m.- 11:30 p.m.

Essential Duties and Responsibilities

  • *Coordinate with community referral sources to screen clients for services
  • *Complete enrollment/intake on eligible clients
  • *Assists clients in the identification of strengths, needs and goals. Develops an individualized service plan in cooperation with the client, include a plan to find and/or keep permanent housing.  
  • *Provides problem solving and crisis intervention services to empower clients to develop self-sufficiency.
  • *Screen for mental health and substance abuse disorders and provide linkages to appropriate services
  • *Assist clients in locating, obtaining, and retaining suitable housing though housing search and placement services
  • *Process requests for one-time financial assistance, as available, to assist in acquiring housing
  • Complete SOAR training and assist individuals in accessing SOAR services either through referral or direct provision of services
  • *Ensure required information is collected, entered into prescribed formats, and reported in a timely manner.
  • *Provides program information and referral services to clients.
  • *Demonstrates knowledge of current community and HFS programs and resources to meet client needs, including providing connections to: mainstream benefits, income support services and Integrated Health Home services.
  • *Utilizes trauma-informed care, person centered and culturally competent principles in providing services to clients.
  • Evaluates efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care.
  • Is able to work both independently, with little supervision, and as a part of a team.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • Able to evaluate program services and make recommendations.
  • Develops personal and program related skills through participation in internal and external training   opportunities including printed material and audio and/or visual media.
  • *Essential functions of this job is to be performed on company physical work site.
  • Performs other program related duties as assigned.

Job Requirements

MINIMUM QUALIFICATIONS 

  • High School diploma required.
  • Bachelor’s Degree preferred or Master’s Degree and no experience
  • Two years experience in a 24 hour care setting
  • Mental Health or Alcohol/drug abuse treatment experience preferred
  • Valid Driver’s License and acceptable driving record
