Job Description

Partners in Empowerment and Recovery (PIER) is a collaboration of CenterPointe and Lutheran Family Services of Nebraska, is an intensive community-based program for severe and persistently mentally ill clients.
We are seeking a full time member of the PIER team to assess, develop, coordinate and provide treatment, rehabilitation, and support services to clients with severe and persistent mental illnesses (SPMI). 
Work as part of a multi-disciplinary treatment team providing individualized, creative and strength-based wraparound care which adheres to fidelity based assertive community treatment standards.

Provide a range of clinically indicated services including but not limited to individual psychotherapy, family therapy, and case management services for adults/older adults with behavioral health and co-occurring disorders.

Assess, monitor, and follow up with client’s psycho-social/mental functional ability to meet basic needs to move toward recovery. Provide monitoring of mental health and substance use treatment issues, medication observation and overall functioning to assess for decompensating and to provide strategies to help client to increase functioning and remain in the community.

Develop and monitor treatment plans; provides documentation of activities in electronic health records. 
Individual therapy, crisis management, and family collaboration of families for program clients

  • Conduct client assessment and ongoing reviews of client progress.
  • Use evidence based clinical strategies to promote change and focuses on behavior change through strategies that improve level of functioning.
  • Provide 24 hour on call crisis service and respond to crisis situations and make appropriate interventions.
  • Provide comprehensive psychosocial services that include a continuum of services designed to improve or maintain client ability to function, including case management services, Develops social skills, interests, and leisure time activities to provide a sense of participation and personal worth.

Job Requirements


  • Master’s degree in Human Service field from accredited college
  • Minimum of State of Nebraska PLADC/PLMHP licenses required, LIMHP preferred
  • Demonstrated ability to provide quality program interventions and coordination to mental health and chemically dependent clientele
  • Valid driver’s license, proof of liability insurance and ability to drive for organization business
  • Availability to work on evenings and weekends on rotating basis
  • Availability to be on-call
